DDS CareNet

DDS CareNet is a national dental Preferred Provider Organization (PPO) owned and operated by Meritain Health.

With participating dental providers in over twenty-six states, DDS CareNet's recruitment philosophy is different than most other networks. Our recruitment focuses on providers identified as highly utilized by our clients.

If your dentist is not currently participating in the DDS CareNet dental network, please complete the Provider Recruitment Request Form.

To Join Our Network

Thank you for your interest in joining the network of participating dental providers in DDS CareNet.

The application process includes completing two documents - the Provider Application and the Participating Dentist Agreement. In addition, certain attachments are to be included with your completed Provider Application.

To make your application as easy as possible, we have made available the Provider Application and the Participating Dentist Agreement:

Remember to print both the Provider Application and the Participating Dentist Agreement and return the signed originals to us.
