When submitting a Request for Proposal via e-mail to sales@meritain.com, please include the following information:
- Your name and contact information
- The company name for whom you're submitting a request
- The proposed plan effective date of the company
- The number of eligible employees
- Please also include all relevant attachments including, but not limited to the: census, claim history, questionnaire
